Phone Order:

• Call 1-800-368-2078 to order the eTelemet service.
• An eTelemet Sales representative will gather all pertinent information.   Before your account can be opened, a signed agreement and any applicable exchange forms must be completed and returned to Telemet via fax (703-684-7374) or via "snail mail." These forms will be provided to you by your eTelemet Sales representative (or follow the links above to retrieve them online).

Mail Order:

Assuming you have received a packet of information from an eTelemet Sales representative, please read over the information.

• Fill in and sign the *eTelemet Streaming Market Data Agreement.
• Fill in the *eTelemet New Subscriber Form - SCHEDULE A.
• Fill in and sign the appropriate **Exchange Form(s) (provided by Telemet upon request or online at the link above). Note: Private Investor exchange forms can be faxed.
• Submit a payment either by check (made out to Telemet America, Inc.) or credit card. If paying via credit card, user must fill in and sign the "Payment Information" section of the eTelemet New Subscriber Form - SCHEDULE A.
